Notary service at a care facility
Residents of nursing homes and assisted living facilities may need a notary for prepared powers of attorney or other simple documents. A mobile appointment can reduce the need to transport the signer elsewhere. Ann considers facility visits in Conyers by appointment, subject to the facility’s access rules and the resident’s participation.
Coordinate before the appointment
Ask facility staff about visitor access, a suitable meeting place and a time when the signer is available. Identify the person who will sign, the type of document and any witnesses required by its preparer. Arrange identification in advance; do not assume that a relative’s ID can substitute for the signer’s identification.
Participation is essential
The resident must willingly participate in the notarization. A family request does not by itself authorize signing for another person, and a notary cannot resolve concerns about a document’s legal effect. If identification, awareness or willingness is in question, notarization may not proceed. Discuss document preparation and legal questions with an attorney.
Fees and related hospital visits
Facility access and the travel fee are discussed before booking. An additional $20 convenience fee applies to hospital and nursing home visits. For assisted living visits, ask Ann to confirm the exact fee before booking. Ask Ann for the exact quote for your facility. Wills are not offered. Prepared powers of attorney and general documents are the main focus.
Standard travel & convenience: $45 within 10 driving miles of Conyers City Hall, plus $2 per notarial act. Hospital and nursing home visits add $20. Farther travel is quoted individually. Fees and availability are confirmed before booking.
Need a witness?
Any document requiring witnesses can use this service. Ann can provide one or two witnesses for $35 each ($70 for two), in addition to travel, notarial and any hospital or nursing home convenience fees. Confirm the required number and eligibility of witnesses with the document preparer. Request a witness on the quote form so arrangements can be confirmed before your visit.
